Dementia support: the role of the case manager

When someone - and their loved ones - are told they have dementia, a lot comes at them. Often, questions arise immediately: What does this mean? What is in store for us? And where can we turn for help and support? During this period, people regularly feel insecure and seek guidance.

A dementia case manager plays an important and trusted role in this phase. The case manager supports not only the person with dementia, but also the family caregivers and loved ones. She offers guidance throughout the process, provides information and education about dementia and helps make choices that fit the family's situation and wishes. In addition, the case manager coordinates the care and support needed and makes connections with appropriate agencies.

This evening will focus on the role of the case manager and there will be room for explanation, recognition and asking questions.
